Form I-90, application to replace a permanent green card.

Purpose of this form is to replace a lost or stolen green card. If your card has already expired, or will expire in the next 6 months, you should go to your local INS office to file a green card renewal.

Form I-130, petition for alien relative.

A citizen or lawful permanent resident of the United States may file this INS form to establish the relationship to certain alien relatives who wish to immigrate to the United States. A separate form must be filed for each eligible relative.

Form I-134, affidavit of support.

Purpose of this form is to file a promise with INS to support an alien entering the United States and that the alien will not become a public charge. A separate affidavit must be filed by the sponsor for each person. If the sponsor is in the United States, the affidavit may be sworn to or affirmed before an immigration officer, or before a notary public or other official authorized to administer oaths for general purposes. If the sponsor is outside the United States, the affidavit must be sworn to or affirmed before a U.S. consular or immigration officer.

I-140, immigrant petition for alien worker.

The I-140 INS form is used to petition for an alien worker to become a permanent resident in the United States. This form is filed on behalf of an alien, usually by an employer.

I-485, application to register permanent residence or adjust status.

The I-485 INS form is submitted by the individual wishing to obtain permanent resident status. It is generally filed with supporting evidence, and may be filed in conjunction with several other applications or petitions.

Form I-751, petition to remove the conditions of residence.

This immigration form is for a conditional resident who obtained status through marriage, and needs to apply to remove the conditions on his or her residence.

N-400, application for INS naturalization.

Purpose of form is to apply for United States Citizenship.
